Did safety warnings go unheeded?

Just last year saw the completion of an £8.6 million refurbishment of Grenfell Tower. The work involved the installation of exterior cladding around the building and the provision of new windows. But could that very refurbishment have played a role in increasing the fire risk at the block? Tonight the Government said that checks would be carried out on blocks undergoing similar refurbishments.
