An inquest into the death of an 18-year-old who was an inpatient in a mental health hospital has identified multiple failures which contributed to her death.
Tia Wilson took her own life while being detained under the Mental Health Act as an inpatient in Maudsley Hospital in South London in 2021.
The charity Inquest, which investigates deaths in state care, has voiced wider concerns that lessons aren’t being learnt, while Channel 4 News has also been made aware of at least five other cases of inpatients taking their own lives at the trust since 2018.
