A ‘seriously unsafe culture’ in the Devon & Cornwall police firearms licensing unit allowed a “fundamentally flawed” decision to return a shotgun to a man with a known history of violence, an inquest jury has concluded.
Jake Davison killed his mother and four other people, including a three-year-old-girl, in 2021, just weeks after having his gun returned by the police.
The jury also highlighted failings at a national level to learn lessons from previous shootings.
